Are you an adult or young person with Autism?


Healthwatch Darlington would like to hear your experiences of the services in Darlington either as a long-term service user, a newly diagnosed adult, or as a young person moving into adult services.  

To better understand the needs of people with an Autistic Spectrum Condition, it is important that every voice is heard.

The information you provide will help service providers understand your needs and will help to improve services for people with Autistic Spectrum Conditions and their families. 

All information collected is anonymous so no one taking part will be identifiable. Your feedback will be shared with Darlington Borough Council.

If you are a parent/carer, service provider or support worker and are completing this on behalf of someone with an Autistic Spectrum Condition please complete it as the person.

The survey will be open for your views until the 30th September 2017 so please dont miss your chance to have your say.
