National Audit of Dementia (Care in General Hospitals) Questionnaire for Carers
The National Audit of Dementia has been running for 8 years. The carer questionnaire was developed for the last round of audit, which took place in 2016. A panel of carers and patients helped them develop their carer questionnaire. They received questionnaires from 4664 people who look after a family member/friend/partner with dementia in England and Wales. They are extremely grateful for all of the feedback we received - it provided an invaluable voice in the results they presented to the hospitals and the public.
Between June and September 2018, carers, family members, friends and key workers can access a paper carer questionnaire for the audit on the wards in all 196 participating hospitals. They have provided paper copies in Welsh to participating hospitals in Wales. The questionnaire is also available online (in English only) at www.carerq.org.uk.
Anyone who completes the questionnaire will have the opportunity to enter a prize draw to win one of five £50 vouchers for a high street store of their choice. Instructions for entering the prize draw are included on the questionnaire and prize draw entries cannot be linked to questionnaire responses.
