They’d also like to offer a reminder that their Rapid Response Team is able to make an introductory visit to palliative patients at any stage of their illness in anticipation of their service being needed or as part of a patient’s advance care planning. Patients and their families have repeatedly told them how valuable this introduction has been to them.
Referrals can be sent via the Community Hospice office on 01325 254321. If you need to speak to one of the nursing team urgently you can contact them directly on the Rapid Response mobile (07720 947 430) alternatively, as you may be aware, they have added an additional referral pathway so urgent (1 hr response) referrals can now also be made via the iSPA.
