Research questionnaire for Adult Traumatic Brain Injury (TBI) survivors from North East and North Cumbria

A brain injury survivor is currently doing a Masters at Durham university and getting data from adult TBI survivors from North East and North Cumbria.

They are looking for adult sufferers of Traumatic Brain Injury who were injured/and or treated in the North East and North Cumbria to complete an online questionnaire which can be completed in around 20 minutes. Family members/friends can aid with entering answers. Questions can be skipped for any reason and “N/A” options are available for some questions if you think a question is not relevant or you can’t remember specifics. There are also questions where you can explain anything relevant if you want.
