Social Prescribers in Darlington including update on their NEW website!

Social Prescribing link workers in Darlington help to connect people with local community activities, resources and services that can help to improve health and wellbeing.

Link workers give you time to focus what matters to you, and will work with you to co-produce a personalised wellbeing plan. 

**NEW UPDATE**

They are delighted to announce that they have just launched their new integrated social prescribing website (https://socialprescribingdarlington.co.uk/) and wellbeing directory which they hope will contribute towards Darlington PCN’s goal of combating health inequalities by:

  1. Creating a free and accessible online resource to help raise health literacy rates in Darlington
  2. Creating an accessible tool for the VCSE sector and statutory orgs to easily signpost their patients/clients to the treasure trove of online resources and local resources in Darlington
  3. Increasing the number of social prescribing self-referrals
  4. Reducing the risk of people becoming digitally excluded by working with local services to map their digital inclusion resources

The website features:

  1. Over 500 resources (and counting) including online and in-person services, groups and activities.
  2. A language converter for the most commonly spoken languages in Darlington plus common European languages (Arabic, Bengali, Chinese (Simplified), Dutch, French, German, Italian, Pashto, Persian, Polish, Russian, Spanish). We can add as many additional languages as we need.
  3. A secure self-referral form. Only patients registered with one of the 11 Practices in Darlington can self-refer.
  4. A ‘messaging bot’. Users can chat to a real person when a member of the team are logged into the system. Website visitors can also leave messages on the chat bot that is picked up during office hours.
  5. An option for website visitors to contact them if they identify that a service is missing or needs correction.
  6. URGENT support link on every page linked to their local mental health foundation trust’s urgent help webpages. Urgent help services are also individually listed within the wellbeing directory.
  7. An option for external services to register an account and upload their services which will be checked and verified by them before going live on the site.

The website was reviewed by Neasham Road Surgery’s Patient Participation Group who said it was easy to use, beautiful, accessible, a great resource for the town and that they will be recommending it to family and friends.

IN PROGRESS

  1. They are in the process of contacting all uploaded services to request a copy of their service/company logo.
  2. The creation of a blog featuring regular service updates and local news.
  3. Audio or video versions of FAQs, Client Success Stories, how to access their services and what to expect during an appointment.
